Scandinavian study suggests there are five types of adult-onset diabetes

Among the severe forms, there was one group with severe insulin resistance and a significantly higher risk of kidney disease than the other types (cluster 3: severe insulin-resistant diabetes, affecting 11-17% of patients), and a group of relatively young, insulin-deficient individuals with poor metabolic control but no auto-antibodies (cluster 2: severe insulin-deficient diabetes, affecting 9-20%).

But what are these different types, and why did the researchers make this decision? Clusters 3 and 4 can be thought of as falling between the two extremes. It now affects 3.7 million people in the UK.

They also did genetic analyses, and compared disease progression, treatment, and development of diabetic complications for each type of diabetes.

New Brunswick is giving more attention to diabetes prevention and care by increasing the age limit to avail funding for diabetes-related supplies like insulin pumps.

Doctors now treat diabetics as either type 1 or type 2.

Lead author of the study Professor Leif Groop from the Lund University Diabetes Centre in Sweden said early treatment is vital for diabetics.

Severe insulin-resistant diabetes patients, generally overweight and producing insulin, which their body was no longer responsive to. To demonstrate their argument, they analyzed health data from almost 15,000 Swedish people with type 2 diabetes, focusing on six variables that had been measured and recorded at the time of their diagnosis: age, body mass index, the presence of beta-cell antibodies, level of metabolic control and measures of beta-cell function and insulin resistance.

Other researchers caution that the team's results need to be replicated elsewhere, using even more diverse patient samples. Current diagnostics and classification of diabetes are insufficient and unable to predict future complications or choice of treatment'.

The researchers said type one should be renamed "severe autoimmune diabetes".

The study involved four cohort studies of 14775 adults who had been recently diagnosed with diabetes across Sweden and Finland.

Severe insulin-deficient diabetes (SIDD): GADA-negative but otherwise similar to SAID.

Cluster 4: Mild obesity related diabetes, seen in 21.6 per cent of patients.

Cluster 5 (n = 3,513) was described as mild, age-related diabetes with older patients but only modest metabolic derangements, according to researchers. Identified in 39% of the people in ANDIS. The study found that people with this type most commonly developed eye disease.

How did the researchers interpret the results?

At present, according to Diabetes UK, of those diagnosed with diabetes in the UK, 10 per cent have type 1 and 90 per cent type 2, but these new sub categories could explain why some patients respond well to certain treatments while others do not.

This is a valuable study that suggests the diagnosis of diabetes may not be as simple as just types 1 and 2.

Today, about 425 million people around the world have diabetes. He said that this would lead to more precise diagnosis and treatment of diabetes tailored to individual needs. He mentioned a caveat saying that this study cohort was a uniform Scandinavian population and thus all races and ethnicities were not adequately represented.
